Fee-for-Service Nurse Practitioner

FEE-FOR-SERVICE

Nurse practitioners have a wide range of areas where they can work. It’s important to note that nurse practitioners in Ontario are not covered by the Ontario Health Insurance Plan (OHIP). This means they charge a fee-for-service when they work independently because they cannot bill OHIP for their work. Below is a list of fees that apply. Since it is a fee-for-service model, nurse practitioners do not work based on the “one issue per visit” policy. However, patients will be billed for the time and expertise provided. Supporting independent nurse practitioners means supporting small businesses.

The benefit of having a fee-for-service healthcare provider is that you can often address multiple concerns in a single visit without waiting for another appointment.

During the initial visit, 45 minutes are charged, and subsequent visits are charged for 30 minutes. Additional time is charged per half hour.

All laboratory tests and diagnostics are covered under the Ontario Health Insurance Plan. Patients are responsible for payment of services and time.

Registration Fee: $400 (+HST) Per Person Billed Annually.

The registration fee will include:

  1. First-time assessment and treatment plan.
  2. Unlimited Video Appointments
  3. Onsite Lab Testing
  4. On-Click Prescription Refills
  5. On-time Appointments

Follow-up visit: $75 (30 minutes)

Follow-up visit: The follow-up visit includes any assessment and treatment plan follow-up based on the previous visit.

During an Episodic Health Visit, the healthcare provider will address the patient’s specific health concern(s), referred consultation, or any identified medical condition(s). This meeting is intended to be a one-time consultation and will cost $75 for up to 30 minutes. There is no expectation of an ongoing therapeutic relationship in accordance with the standards of practice.

ADHD testing: $400

Mental Health Services: $150

Sick notes for work /school/daycare: $30

WSIB paperwork: To be billed to WSIB (may include physical examination)

Driver’s medical form: $150

Pre-employment examination and form only: $100

Pre-employment examination + physical exam +/- injections: $150

Fitness Medical for camp, daycare, air travel, etc: $45

Disability parking form: $50

Prescription refill by fax or phone: $30

School/College/University Medical Certificate: $50

Unemployment insurance/Sickness benefits: $30

Other paperwork: at the discretion of the healthcare provider.

Fees are followed by the most recent recommendations of the College of Physicians and Surgeons of Ontario and the Ontario Medical Association.

**Prescription renewals usually require an in-person appointment to evaluate the condition necessitating the prescription. If you cannot schedule an appointment for a medication review, there will be a fee for this service.

If nursing services are covered under private benefits, the fees for those services may be included in the coverage.
